A network of clinical operators,
backing BC clinics.
Not a solo consultancy with a slide deck. A collective of clinicians, digital-health leaders, and operations experts — backing one practical brand for clinical practice.
TOSC
The Other Side of Care
(n.) The unseen work after the visit — chasing results, fixing routing, replying to messages, finishing notes. The side that steals evenings, adds clicks, and slows access.
Our job: take those tasks off the clinician’s plate so they can get back to patients.
Not one person. A collective.
TOSC is a network of clinical operators, digital-health leaders, and operations experts — backing one practical brand. The breadth is what lets a single practice draw on the same caliber of thinking as a multi-site network.
Clinical operators
Former clinicians who moved into operations — physicians, physios, chiros, RMTs, counsellors who know what the clinic floor actually feels like.
Digital-health leaders
EMR implementation leads, AI roll-out specialists, workflow designers. The people who've built the systems clinics actually use.
Operations experts
Strategy and operations consultants with experience in global consulting, public-sector health programs, and fractional leadership.
Problem-watching, not product-planning.
TOSC started as a newsletter — a place to advocate for change and surface the pain points clinicians were living with but the industry wasn't naming.
Writing became doing. Clinics asked for help with the problems the newsletter surfaced. Consulting engagements followed.
Code joined consulting — a small library of apps for the places where custom software is cheaper than consulting hours. Performa, the AI form-writer, shipped first.
One practical partner with a network of clinical operators behind it. Independent clinics to multi-site networks, across BC and beyond.
Three commitments.
Applied every engagement. No exceptions.
Done alongside your team.
No deck-ware. The team that will run it is the team we build it with.
Built to be left behind.
Every engagement has an exit plan on day one. You own the outcome.
Patient data stays in the browser.
Local AI, Canadian infrastructure. The trust posture is the architecture.
The founder.

Himanshu Khetarpal
Founder · Network lead
Himanshu founded TOSC in 2023 after fifteen years across four BC health authorities, global consulting at IQVIA and Deloitte, and the Provincial AI Clinical Documentation roll-out.
He currently also serves as Regional Manager, Health Information Management at Providence Health Care in Vancouver — a day role that keeps TOSC grounded in real clinical-operations practice. The two roles are complementary, and TOSC’s engagement cadence respects both.
Rooted in Vancouver. Reach spanning four continents.
TOSC is headquartered in BC. The network’s clinical and operations experience is drawn from the cities below — applied locally, one clinic at a time.
