About
Healthcare from the floor to the province.
TOSC is led by Himanshu Khetarpal, who has spent fifteen years seeing healthcare from every side: the clinic floor, the global consulting boardroom, and the operating core of a health system. The range is the point. Very few advisors have all three.

The Other Side of Care
The other side of care is the human one. The unhurried conversation, the attention, and the clinical judgment that made people choose this work in the first place.
Admin is what crowds it out, a little more every year. TOSC exists to take that weight off, so the people who deliver care can spend their time on the side that matters.

Three vantage points most advisors never get.
The same problem looks different from each one. He has worked all three.
From the bedside
He started as a clinician, treating patients. So he knows first hand what the admin actually costs the people doing the work.
From the boardroom
He advised governments and funders with Deloitte and IQVIA, designing health-system programs backed by the Gates Foundation and the Global Fund across three continents. So he knows how the big decisions get made, and paid for.
From inside the system
He built a primary care network, worked on a $1.5B transformation, and ran an ambient-AI documentation deployment inside a health authority. So he knows how change actually lands inside a real health system.
Career background
AI deployment, from the inside
He led a health authority's delivery within British Columbia's AI clinical documentation program — Canada's first multi-vendor, multi-site evaluation of ambient scribes in a publicly funded health system — and built the local governance, privacy assessment and adoption model it ran on.
Few advisors have actually deployed clinical AI inside a real health system. That is the difference between advice from a deck and advice from the seat.
The firm
Deliberately small. That is the design, not the stage.
Large firms run a leverage pyramid: partners sell, juniors deliver, and the client pays for the training. TOSC is built the other way. The person who scopes your engagement is the person who delivers it — with specialist help brought in from a trusted network when a job genuinely calls for it, never as a substitute.
Independent since 2026, and structured to stay conflict-free. TOSC is a private practice: it works with clinics, allied health groups and Divisions, and does not contract with health authorities or government. No software to sell, no vendor commissions, no alliance targets. What a large firm spends on overhead, we put behind the guarantee — if the agreed metric doesn’t move, the engagement isn’t billed.
How he operates.
Independent
No software to push, no vendor kickbacks, and no government or health-authority contracts. Three things most advisors cannot say at once. The only thing being sold is your result.
Advise and build
Most advisors stop at the recommendation. He can deliver the thing he proposes, then hand it over.
Honest about the work
He will tell you what to leave alone as readily as what to fix. A short, true plan beats a long, impressive one.
